Caregiver Ombudsman Outreach Program

If you live in Inwood, Washington Heights, Central or West Harlem (Community Boards 9, 10 & 12) and need assistance coping with the demands of care-giving and need support, please know that you are not alone; there are services available that can help lighten the load.

The Caregiver Ombudsman Outreach Program (Co-Op) will identify caregivers and link them to available resources and information.

Caregiver support services include:

  • Needs assessment
  • Advocacy
  • Information and referral
  • Home visits
  • Counseling
  • Arrangement for Home Care Aides
  • Assistance with Household Chores
  • Other Services as needed

It is our hope that through the support of the Caregiver Ombudsman Outreach Program (Co-Op), northern Manhattan caregivers will become more knowledgeable, confident and competent in their challenging roles; better able to manage stress and access the help they might need.

The Co-Op Program represents the ever-growing efforts of Isabella and area partners to create innovative opportunities to serve senior residents and their families in Upper Manhattan.
