Residents in our Short-stay Rehabilitation unit receive a targeted, intensive program under close supervision of our multi-disciplined Care Team.
Isabella offers personalized, rehabilitation services for those requiring short-stay sub- acute care. Isabella’s Rehabilitation Program encourages independence and a rapid and safe return home. We also offer education to help each person improve his or her well-being and make healthy lifestyle choices with the ultimate goal of reaching the highest level of function.
Our therapy programming encompasses the latest rehabilitation technology and modalities in addition to innovative treatment plans that include both individual and group therapy up to 7 days a week.

In addition to our fully equipped rehabilitation suite, we have a greenhouse available for horticultural therapy. We also offer a prosthetic program for amputees, who once they are cleared, can be fitted for prosthetics then get follow-ups and adjustments.

We Evaluate Each Person's Needs
A care plan will be designed to meet each person's rehabilitation goals, based upon a professional evaluation. A multidisciplinary team comprised of a doctor, registered nurse, social worker, therapists and specialists appropriate to your specific condition will work with you in achieving these goals.
Each resident will begin therapy sessions with an individual therapist upon admission. The therapist will establish a customized program to meet each person's needs and will work with him or her to attain personal goals.
Our specialized rehabilitation nursing staff will make sure that each resident has the skills needed to manage daily life beyond the rehabilitation unit, for example:
- Assistance with daily living skills, such as meal preparation retraining and tub transfers
- Use of adaptive equipment
Professional staff will work with residents and their families to develop a discharge plan that supports a comfortable and safe life at home.
